Please complete the form below to request reasonable accommodations for programs and events at UNC athletic facilities. Accommodations will be provided to the extent possible as determined by availability, the individual's request, and UNC's assessment of individual needs.
You may complete the Accessibility Request Form electronically here. Alternatively, you may print the form (.pdf) and mail it in.
Patrons who wish to request auxiliary aids and services should contact the UNC Department of Athletics and submit the completed Accessibility Request Form at least two weeks prior to an event. However, regardless of when an Accessibility Request Form is submitted, UNC will make a good faith effort to fulfill requests made via this form. UNC staff will consult with the submitting party to determine what aids or services are necessary to provide effective communications.
Accessible seating is reserved exclusively for UNC's patrons with accessibility needs and up to three companions, all of whom have already purchased tickets (standard admission).
Please note that UNC is not required to provide accessible seats in the event of a sellout. Guests who need accessible seats have no greater right to obtain tickets to a sold-out game or event than do guests who do not require accessible seating. Completion of the Accessibility Request Form does not entitle a patron to purchase tickets to an event or game and will not be considered evidence of a patron's purchase of tickets or a patron's request to purchase tickets for a game or event.
The purchase or use of accessible seats by individuals that do not need accessible seating is strictly prohibited and can result in ejections and/or revoked ticket privileges without refund.
By submitting the Accessibility Request Form to the UNC Department of Athletics, you are verifying that you intend to abide by the policies and guidelines of the University of North Carolina at Chapel Hill and its Department of Athletics, that the information regarding your needs and all other information provided for the completion of the form is true and accurate, and that you agree that UNC has the right to eject you from the game, program, or event and/or revoke your ticket privileges without refund, if it becomes apparent that tickets are not being used by the person(s) for whom they are intended or for their intended purpose (i.e., accommodation of individuals with disabilities or chronic medical conditions).
