Agents/Advisors wishing to represent UNC student-athletes must register with both the State of North Carolina and the UNC Department of Athletics prior to contacting the student-athlete(s). To initially register or to renew your registration (required annually) with the UNC Department of Athletics, please complete the applicable form and return it to the address below:
